We serve adults 18 and older who are rebuilding after jail, treatment, or time without a home. Wherever you are starting from, we meet you there and stay with you as you move forward.
Our team is led by people who have lived it, and that changes everything about how we show up for you.

A place to stay
Safe, structured housing with people around you who understand. A stable place to land is where everything else starts.
Support to stay well
Recovery coaching from people who have been there, plus real tools to protect your progress and your sobriety.
Skills for the next step
One-on-one coaching on goals, money, work, and school, including GED and college. Practical help to build the life you want.
Help getting connected
We connect you to doctors, mental health and recovery care, and county resources, and we help you get there. You won’t have to figure it out alone.
OUR HOMES
A safe place to call home.
Our supportive homes are where stability starts. They are real houses in the community, not facilities, where people rebuilding their lives live alongside others who understand the road they are on.
Each home is structured and substance-free, with clear house guidelines and the steady presence of peers and staff who have walked the same path. That balance of structure and support is what makes lasting change possible. Residents share meals, chores, and milestones, and over time a group of individuals becomes a community. It is the difference between simply having a place to sleep and having a place to belong.
Structure and accountability: Clear expectations and a substance-free environment that protects everyone's progress.
Peer support: Living alongside others in recovery, guided by staff with lived experience, so no one rebuilds alone.
Dignity and belonging: Real bedrooms, shared meals, and a stocked pantry, the everyday things that make a house a home.
HAVE MEDI-CAL?
If you have Medi-Cal, we can do even more.
If you have Medi-Cal, you may qualify for extra support at no cost to you. Opportunities For Change is an approved CalAIM provider, which is a California Medi-Cal program, so we can offer this help directly. That can mean one person to help manage your care, help getting and keeping housing, and a guide who checks in and walks beside you.
ENHANCED CARE MANAGEMENT (ECM)
You get one person, a care manager, who helps you keep track of appointments, medications, housing, and health, so you are not doing it all alone. They show up, follow up, and help the pieces fit together. If you have Medi-Cal, this is free.
This might be for you if:
You are leaving jail or prison
You are staying in a shelter, a car, or on the street
You are working on your mental health or your recovery
You end up in the hospital or ER more than you would like
Someone in your corner.
COMMUNITY SUPPORTS (CS)
Help finding and keeping a home.
Housing is health. Through our Community Supports, we help you find a place, cover the cost of moving in, and keep it once you are there.
Finding a place. Help searching, applying, and getting ready to move in.
Moving in. Help with deposits and the basics to get set up.
Staying housed. Ongoing support so you can keep your home for the long run.
COMMUNITY HEALTH WORKERS (CHW)
A guide who has been there.
Our community health workers are trusted people from the community, many with lived experience of their own. They answer questions, help you find your way through the system, and stay in touch, so getting care feels less overwhelming.
